Funding for community services

As part of a £14 million national NHS programme, south east London has been chosen as one of seven areas to design and deliver an enhanced community service for older people and adults with complex needs.

 

From April, our rapid response teams will begin to develop services so that we can respond to referrals from a GP, other health professional or social worker within two hours.

 

The aim is to support more patients at home and avoid unnecessary hospital admissions, building on our experience in Lambeth and Southwark and working with colleagues across south east London.
