Community Diabetes clinic

Due to current COVID-19 situation the Diabetes Community team have made some changes within the service and will be prioritising patients identified with poor glycaemic control i.e. Hba1c>75mM/M (or troublesome hypoglycaemia) who may be at high risk of decompensation in the event of COVID-19 infection.

Therefore clear referral information and record of blood glucose monitoring is essential to enable us to advise and support patients appropriately.

 

Southwark community diabetes team.

1-2 community diabetes nurses working Monday-Friday

Community clinics twice per week (Wednesday Dulwich and Friday Bermondsey) with GPSI cover.

Mostly telephone appointments but face to face appointment available for urgent cases e.g. insulin starts

Refer via e-RS from primary care or usual email from secondary care.

 

Lambeth community diabetes team:

1-2 community diabetes nurses working Monday-Friday

Community clinics twice per week (Tuesday Gracefield, Thursday Norwood) with GPSI cover.

Mostly telephone appointments but face to face appointment available for urgent cases e.g. insulin starts

Refer via e-RS from primary care or usual email from secondary care.
